This is a Model Release Form. By filling out the form and clicking "submit" below, you grant full permission to Buckhead Studio Photography to all images and recorded media footage taken of the model/talent named below during said shoot production.
Such use includes, but is not limited to the display, advertisement, marketing, distribution, publication, transmission and general use of all forms of images and media to be used in print, digital use and social media. I further state my permission is voluntary. I will receive no (further) financial compensation in relation to produced images or media. I waive any right to inspect or approve images or media in original, edited, composition, or altered form. I release Buckhead Studio Photography and assignees, licensees, successors in interest, legal representatives, and heirs from any current or future liability connected with any images and/or media. This permission applies to individual named in the "Model/Talent Name" field of this form, for which, I have legal authority to sign for.
I hereby release Buckhead Studio Photography (and Shannon McCreary, photographer) from all claims and liability relating to any imagery, video or photographs taken of me on production shoot date provided below.
